“GOOD FAITH ESTIMATE” DISCLAIMER
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance, (this applies to private pay/out of pocket clients), an estimate of the bill for medical items and services.
As of January 1, 2022, all healthcare providers are required to provide estimates for the cost of your care. You are entitled to receive a copy of your Good Faith Estimate (GFE) when you schedule an appointment and/or upon request. The GFE shows the cost of services that are reasonably expected for your healthcare needs and treatment if insurance is not used or does not cover the service. This protects you from balance billing and other unexpected charges.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
NutriTrain, LLC will provide you a GFE upon request. Please note that this GFE does not include unexpected costs that could arise during treatment, including cancellation, additional lab requests or no-show fees.
This law requires healthcare providers to supply you with an estimate of the costs for your nutrition services at NutriTrain, LLC. While it is not possible for the dietitian to know, in advance, how many sessions may be necessary and appropriate for a given person, this document provides an estimate of the cost of services provided. Your total cost of services will depend upon the number and length of sessions you attend, as well as your individual circumstances.
NutriTrain, LLC will provide an estimate of costs upon scheduling and/or as requested. Unexpected costs may include fees for outside referrals, cancellation and/or no-show fees and lab work.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
